Join a compassionate and patient-focused Home Health team! We are looking for a Home Health Nurse Office Case Manager (LVN or RN) to join our expanding organization. In this role, you will assess, diagnose, and treat assigned patients, collaborating closely with our dedicated field nurses.

REQUIREMENTS:

New grads are welcome!
Valid CA Nursing license
Excellent customer service and interpersonal skills
Strong problem-solving and critical thinking abilities
Ability to thrive in a fast-paced environment
Proficiency in computer skills
