Department: Home Care Organization - Field Staff
Reports to: Director of Operations
Schedule: Available as Part-Time, Full-Time, or Per Diem
Location: This position requires field visits. candidates must be located in and around Rockingham County

Job Summary:

As a Registered Nurse (RN), you will be responsible for providing compassionate and personalized care to clients needing assistance with daily living activities, ensuring their comfort, safety, and well-being. Working under the supervisor of the Clinical Director or Director of Operations, you will support clients by assessing their functional status in the areas of social, mental, and physical health, economics, and activities of daily living. You will be responsible for initial assessments and creating the Plan of Care that aligns with the patient’s needs upon admission to Home Care Organization services. You will be responsible for working in conjunction with the Licensed Practical Nurse, Supervisory visits for Home Health Aides and Personal Care Service Providers, ensuring their tasks are aligned with the patient needs documented in the Plan of Care. At any time if you identify a change in status or problems within the home, this needs to be reported to the Director of Operations, Clinical Director and if needed reported to the patients Case Manager and Primary Care Physician.

Educational and Professional:

  • Associate degree in nursing from an accredited school of nursing; Bachelor Degree preferred
  • Valid License as a Registered Nurse in the State of New Hampshire
  • One (1) year of Medical/Surgical experience as a Registered Nurse.
  • Home Health experience preferred, but not required.
  • Reliable transportation and the ability to travel from place to place.

Responsibilities:

  • Makes initial assessment of client’s condition, sets up a plan of care with client, implements this plan in coordination with MD, and documents all.
  • Establishes treatment priorities based on medical diagnosis and implements them with the client’s care and safety in mind.
  • Prepare medication boxes, assess treatments and educate as needed.
  • When appropriate, work with clients to gain an increased level of independence with treatments.
  • Works with other community agencies involved with the client.
  • Observes signs and symptoms, reactions to medication and/or treatments and changes in the client’s physical or emotional condition and reports to the Clinical Director as needed.
  • Reports significant changes to client’s physician.
  • Teaches, supervises, and counsels the client and/or family members regarding the nursing care needs and other related problems of the client at home as well as providing education for the client and/or client’s family through individual teaching programs.
  • Supervises, directs, and evaluates the performance of the Licensed Practical Nurse, Home Health Aides, Personal Care Service Providers, ensuring that the client’s personal hygiene, grooming, comfort, and safety needs are met, and all is documented.
  • Makes recommendations for additional or modified services, coordinating client’s total needs with other members of the health care team.
  • Ensures established inventory standards for medicines, controlled drugs, other supplies, and equipment are maintained.
  • Participates in the nurse on-call system.
  • Participates in staff development programs and meetings as assigned.
  • Assists in the orientation of new employees to the home environment.
  • Assists in the ongoing competency program for all staff members.
  • Prepares client and/or client’s family for discharge from the home care program.
  • Demonstrates self-improvement and continuing education efforts and meets all state and federal educational requirements and mandated in-services, as well as keeping RN licensure current.
  • Completes, in a timely manner, all required paperwork and documentation including client records, time sheets and required reports.
  • Works with appropriate level of independence, recognizing limits and seeking proper supervision.
  • Demonstrates dependability and punctuality.
  • Demonstrates adaptability to change and is able and willing to learn.
  • Participates in Agency quality programs and actively seeks ways to improve patient care and the Agency’s efficiency.

Physical Demands:

  • Lifting, carrying, pushing, or pulling up to 50 pounds
  • Patient transfers with no assistance for direct patient care
  • Standing, walking, stair climbing, stooping, kneeling, and crouching while administering patient care in the home
  • Reaching and handling
  • Adequate eyesight for reading, writing, driving, and performing tasks

Aptitudes:

  • Ability to understand verbal and written instructions.
  • Read and write documentation in patient record.
  • Work with the hands and fingers to manipulate large and small objects.
